Senior Phase – Transitions Programme

Transitions is a revised programme for young people who are at risk of entering a negative destination upon leaving school. Young people are supported into a positive and sustained destination with aftercare support in line with the Opportunities for All Commitment.  The Transition service has 6 x Officers and is supported by Youth Guarantee.

 

Young people in the Senior Phase and their last year of education and attend a West Dunbartonshire School are eligible for this support. Schools and Skills Development Scotland (SDS) will identify potential young people at the 16+ meetings using the 16+ Risk matrix and local discussions. Places will be allocated following the receipt of a completed Early Transitions Referral. Referrals can also be made out with the 16+ meetings. The aim of the Transitions Programme is to support young people who are still attending school but are at risk of becoming NEET and need the most support to move onto a positive destination and also for some young people who have completely disengaged from school.

A designated DYW School coordinator will support the young person to agree an action plan of bespoke activities and learning which will include barrier removal and engagement.

 

Activities are not prescriptive but can include employability skills, volunteering, health & wellbeing interventions, personal & social development, social group, other group work, wider achievements, employability tasters and online courses.

It is not for every young person and is targeted to those who have disengaged and require a supported transition to engage with SDS and post school opportunities and are assessed as a suitable referral for a post school Learning Agreement. It is a service to support transition, and is not an alternative to Education. It is flexible to fit around any subjects and/or SQA awards the young person is working towards. For those young people who have completely disengaged, we can support a partnership approach to reengage them with school where possible and/or participate in activities outside of the school setting.

 

The role of the service works with partners to assess the young person’s employability needs, using assessment paperwork to develop an action plan with agreed outcomes. This should align with any other plans in place for the young person.

 

The Senior Phase Officer maintain regular contact with Pastoral Care, DHT and SDS School Careers advisors. We share information with post school partners on the young persons progress in order to plan for progression to a sustained positive destination.
